Referral
Procedures
In
view of the duties of the referring dentist given the current
GDC guidance we advise that referring colleagues allow us to
establish the Justification for a general anaesthetic, and to
take the responsibility of prescribing a general anaesthetic
where appropriate.
The
Clinic's policy at the assessment visit is to give a thorough
and clear explanation of all anxiety management techniques available,
and a clear explanation of the risks involved.
We
ask all colleagues to allow us to take the responsibility of
prescribing the appropriate anxiety management technique justified
according to our protocols.
All
dentists referring patients to Queensway Anxiety Management
Clinic (QAMC) should be in possession of the following documentation.

Please
contact us if you require additional supplies of the above.
All
patients seen on a referral basis must be matched with a written
or electronic request, preferably using the referral form provided
or via our website (www.anxietymanagement.co.uk). Forms can
be sent by post or be faxed. We prefer, where possible, to assess
patients at a separate visit to their treatment appointment.
However, we recognise that there will be occasional 'acute'
referrals requiring more immediate treatment. For acute referrals,
patient details can be given by telephone but the referral form
must be faxed or accompany the patient to their assessment.
Once the patient's details have been received, they will be
contacted to arrange an assessment appointment.
Action
- Please use referral forms provided enclosing other relevant
records and ensure patients know the location of the clinic.
Patients should bring with them any prescribed medications and
should be informed that the first visit will be for an assessment
only. Alternatively use out electronic referral service through

forwarded to us, we will make every effort to return).
FAILURE
TO ATTEND
If
a referred patient fails to attend the assessment visit a letter
is sent requesting the patient to contact the surgery for another
assessment appointment.
If
the patient does not respond to this letter the referring dentist
will be informed that we have been unable to contact their patient
and have discharged the patient from our care. We are happy
to accept a re-referral.
If
a patient fails to attend a treatment appointment, fails to
comply with the written and verbal instructions that have been
given, or cancels the appointment within 24 hrs, a missed appointment
charge will be made. The charge will reflect the costs of Clinic
staff including the consultant anaesthetist, the treating dentist,
the nursing and recovery staff.
